Empowering Communities: Bridging Health Disparities in the Inland Region
The Health Equity Team at Reach Out is home to a team of community outreach leaders who specialize in providing equitable and inclusive efforts around health disparities throughout San Bernardino and Riverside Counties. We collaborate with community partners and members to ensure the availability of health resources, information, and education to produce more equitable outcomes in our communities.
Our team aims to identify and eliminate health and social disparities heightened or caused by COVID-19 in disproportionally affected communities throughout the Inland Region.
