Community Health
For nearly 6 years, we’ve been working side by side with community members and partners to address the deepest-rooted causes of health outcomes in the Inland Empire. Through programming and partnerships informed by community input, we’re creating a more vibrant Inland Empire that empowers our neighbors with the knowledge and resources to own their health.
Our Programs
Healthy Jurupa Valley
Founded in 2011 during the city’s incorporation, HJV is one of the longest standing and most successful healthy city initiatives in California.
Leadership Academies
A 10-week course that equips community members with knowledge and skills to lead substance use prevention efforts locally.
Medication-Assisted Treatment (MAT)
This program combines medication, counseling, and peer support to help manage addiction, reduce relapse, and reclaim stability in daily life.
Substance Use Prevention Trainings
Interactive trainings focus on substance misuse prevention through trauma-informed education, community dialogue, and real-world tools.
Drug Takeback Programs
Permanent drop boxes and annual take-back events make it easy for residents to safely dispose of medications, reducing risk of misuse.
Latino Health Collaborative
Latino Health Collaborative works to advance health equity by addressing cultural barriers and improving healthcare access for Latino communities.
Partners for Innovative Communities
A regional coalition building healthier communities through policy change, cross-sector partnerships, and grassroots collaboration.
Fontana Community Coalition
A grassroots coalition reducing youth substance use in Fontana through policy, culture, and community-led prevention efforts.
Morongo Basin Community Coalition
MBC brings residents and partners together to reduce substance misuse and promote a safer, healthier Morongo Basin.
California Violence Intervention & Prevention (CalVIP)
As a CalVIP grantee, Reach Out is leading Jurupa Valley’s first strategic plan to reduce violence and support long-term community health.
CalFresh
Reach Out partners with CalFresh Healthy Living to deliver nutrition education, garden programs, and events that promote healthy living.
Featured Initiative
Building on the infrastructure, programming, and best practices we have accumulated over a decade of successful outcomes in Healthy Jurupa Valley, the Inland Healthy Cities Institute was born of the demand to bring these strategies to the rest of the inland region, the Institute is grounded in an understanding of the concerns of the diverse population of the IE, allowing us to bring cultural competence and geographic insight to bear as we work together to cement the Inland Empire into a destination for healthy living and community well-being.
